二氧化碳培養箱(CO2 incubator)

今天收到使用單位送來的請修單，要把二氧化碳培養箱(CO2 incubator)移掉，因為衛生局來稽查，說這廠牌不合格(?)，要移掉，不然會被罰..@@

看了留下來的單子，上面寫，本設備不以醫療器材列管，不得用於人工生殖(IVF)業務....

問題是在衛生署的許可證查詢中，沒有一台二氧化碳培養箱是以醫療器材列管的啊．．．去哪找啊．．．

這引起了我的好奇．．．就打電話過去問是什麼因素呢(Recall還是其他)，這也引發了一連串有趣也有點沈重的想法。

先打去縣衛生局，說是由國民健康保險局來的函，要他們出來稽查，"ASTEC廠牌系列的二氧化碳培養箱"是否用於人工生殖中心，那是不合格，再用要開罰！

要了國民健康保險局該承辦人的電話，在台中，打過去，用意是在提醒她，全國沒有一台二氧化碳培養箱是以醫療器材列管的，如果這様的話，那各家人工生殖中心業務，不就要停了嗎...她回答，她們只針對這廠牌，不知道其他的也都一樣，但這是TFDA回答，所以再要了TFDA承辦人的電話打去問看看原因。

TFDA原本以為我是要來爭為何不能用，所以口氣一直很．．．算了，我是要提醒她，全國沒有一台二氧化碳培養箱是以醫療器材列管的啊，TFDA意思是，那是廠商送件時沒勾IVF，所以當然就不以醫療器材列管，也沒錯啦。

但都沒有列管，我們要去哪買啊，她說那是你們的問題啊，他們只管查核，如果真的都沒有合格的，那可以專案進口啊@@(她大概真的不知道二氧化碳培養箱在人工生殖中心的用途吧，來得及嗎．．．真的是何不食糜乎)，

那其他廠牌呢，沒來問就都算了?我是請他們是否可以發文給各二氧化碳培養箱的廠商，提醒他們重新申請用於IVF，這樣就可以以醫療器材列管, TFDA也不置可否．．．唉，公務員真的是要勇於任事啊。

回報給國健局的承辦人，她也很為難的說，目前是只有查ASTEC這廠牌；她也不知道，竟然所有二氧化碳培養箱目前都是以非醫療器材列管，所以要怎辦呢，呵．．我也不知道，我只提醒她，如果真要全面清查，一定要緩衝時間，不然生育率己經那麼低了，再讓人工生殖中心業務停擺，那不是更．．．．

所以如果你們家也有人工生殖中心，請快確認一下二氧化碳培養箱的廠牌，如果是ASTEC的，先下架吧，如果是其他廠牌的，打個電話去push他們趕快加申請IVF的項目吧，免得橫生枝節哦。

Reason for Recall: During ventilation of small pediatric patients with high airway resistance and low lung function, there may be unexpected high internal oxygen consumption by HAMILTON-T1 ventilators with software versions 1.1.2 and lower. This may cause miscalculation of the required oxygen needed for long time applications with limited oxygen supply. The HAMILTON-T1 oxygen capacity must be calculated using a larger margin than originally expected.
This product may cause serious adverse health consequences, including death.

FDA Comments:
On January 11, 2013, Hamilton Medical mailed a letter notifying their customers of the problem and informed them that the firm is working on a software version with reduced oxygen consumption. The letter included a "Medical Device Safety Alert and Corrective Action" document. The firm asked their customers to review this document for details regarding the root cause and corrective action with the current 1.1.2 or lower software.
Customers will be notified upon completion of the new software. Arrangements will be made to provide a loaner unit while the customer's T1 ventilator is returned to the firm's Reno Service Department for the software upgrade.
The firm also directed their customers to update their T1 Operator Manuals with an included Oxygen Consumption formula page which provides revised information for calculating oxygen capacity.
Class I recalls are the most serious type of recall and involve situations in which there is a reasonable probability that use of these products will cause serious adverse health consequences or death.

SHIMADZU MUX100 No Power

故障情形：No Power

故障原因：電源線捲線器內部斷線

今天這台SHIMADZU MUX100真的是處理得人仰馬翻，如果要寫維修報告，真的只是4個字---> "線斷重接"，但是這4個字耗了3個人快半天的時間，把整台MUX100幾乎都拆開了，才拿到這個"電源線捲線器"，真的，當拆出來時，大家幾乎都快虛脫了。

本來叫修時，看到是No Power，無法充電，因為以前也是有過類似的狀況，大概都是插頭那邊的電線因為長期大力拉扯，而斷線，所以想說，大概沒什麼太大問題，就請工程師直接處理，結果．．推回來辦公室了，看到同事二個人己經拆了大半的外蓋，就加入行列，查看看。

日本的機器本來計上就是很精巧，所以東西都很容是用鏍絲卡在一起，所以如果要拆一個零件出來，大概都要拆下連帶的4,5處，偷懶不得。用電表量，有一條電線不通，本想說，電線前段剪掉一段，看是不是斷在那裡面，結果．．．殘念，

討論後，覺得可能是是斷在那個捲線器的某部份，但是看到捲線器的位置，真的手有點軟，所以就先試看，直接接電，嗯，動作正常了，所以大家看了看，認命吧，從電池組先拆下，看到那個捲線器了，要下來，後蓋要拆，後蓋要近連懸臂底部的蓋板也得拆...終於都拆完了，MUX100，也差不多都拆開了...>_<

終於拆下捲線器，慢慢拉開，大家正要決定從哪剪斷測試時，拉到底，看到元兇了，電源外皮斷了，一條線斷了懸在那，就是你啦．．為了你，幾乎整個MUX都拆解開了。
但是捲線器也是有機關，因為是有彈簧作回接的動力，真的很怕，一彈出來就沒法收拾了，還好，順利解開，重新要接線接端子，再組回去。

4個字"線斷重接"，使用單位可能說，只是線斷重接一下，怎要那麼久，但是3個人，半天的工作，個中甘苦真的是只有同行的人才知道。
